Breach

Important Message for our Patients

Excellent patient care is at the center of our mission at Family Medical Center and Internal Medical Center, and we take seriously the confidentiality of the information we hold.

Our practices, including records from when we owned Family Medical Center in Thomaston before it became a part of Upson Regional, were victims of a data breach when an unknown hacker stole files containing personal information of some of our patients in 2017. We became aware of this theft in November of 2020 when the FBI told us they had found the files on a third-party computer that was not part of our medical practice.

We investigated and were able to determine that the hacker did not access our medical records database and the pathway used by the criminals was found and eliminated during a routine IT check-up several months after the breach. The information that was stolen is limited to affected patients’ names, addresses, dates of birth and social security numbers. The files taken did not contain healthcare records or financial records, including credit card or bank account numbers, and there is no evidence of any further access to or transfer of this information.

Nonetheless, as a precaution we are offering one year of free identity theft protection and credit monitoring to each affected patient. If you are a patient or parent/guardian of a patient of ours, and your information was part of the stolen data, you were mailed a notification letter explaining what happened and how it affects you, as well as what we are doing to protect you. Please read the letter thoroughly so you know more about the incident and how to enroll in the free MyIDCare identity theft protection offered. Please do not discard the letter, as you will need the enrollment code to enroll in your free identity theft protection. If you do not receive a letter and want to confirm whether your information is involved, please call 833-754-1794 between 9 a.m. and 9 p.m. Eastern Standard Time (EST) Monday through Friday.

To enroll in MyIDCare, visit https://response.idx.us/gore or call 833-754-1794 between 9 a.m. and 9 p.m. EST Monday through Friday. When you call that toll-free number, you may hear a brief message and then can choose to be connected to a live operator who will do his or her best to answer your questions.

Additional information can be found at the link below which offers tips and recommendations for other ways you may choose to protect yourself, as well as contact information for the three main credit bureaus. Recommended Steps to help Protect your Information (PDF)

Thank you for your patience and understanding as we continue to provide you with optimum care, and the best information and support that we can.

Sincerely,
